WD Green freezes PC on activity

Hi guys,

I’m having issue with my HDD where it will randomly freeze when I try to write or read any file from the Disk. Can anyone tell me whats wrong with it? I was able to run the WD Diagnostic tool and it only pass with extended test. On quick test it fails.
I only uses this Disk as storage.
Below are the test result.
